This policy covers this website only. It is written plainly on purpose: you should not need a lawyer to find out what happens to your phone number.

What we collect

Only what you type into the contact form: your name, email address, phone number, state, county, and whatever you write in the message box. Nothing else on this site asks you for information about yourself.

Our web host also keeps standard server logs, which include IP addresses and the pages requested. Those are used to keep the site running and secure.

What we ask you not to send

Please do not send medical records, Social Security numbers, or claim file details through this form. It is a website contact form, not a secure medical portal. If we need those documents we will tell you a safe way to get them to us.

Why we collect it, and what we do with it

To answer you. That is the whole purpose. We use your details to respond to your enquiry and, if you are an injured federal employee, to help connect you with a clinic near you that treats federal employees.

We do not sell your information. We do not rent it, and we do not share it with advertisers. Your details are shared with a partner clinic only when that is what you asked us to do.

How long we keep it

We keep enquiries for as long as we are working with you, and for a reasonable period after that for our own records. If you want your information deleted, ask us and we will delete it.

Cookies and measurement

This site does not set advertising cookies. If we add website analytics, it will be limited to understanding which pages people find useful. Educational videos are embedded through youtube-nocookie.com, which does not set a tracking cookie unless you actually press play.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to delete it. Contact us and we will do it. We will not make you fill in a form to make a request about a form.
